What you need
In order for our staff to accurately direct-bill your insurance plan or third-party payer you will need your 2 identifying numbers on your insurance card (names of which vary with each insurance company eg. ID, group number, certificate number, etc.).
We are only able to direct bill to separate insurance plans if they belong to the same company. If your plans differ you can still send in the remaining balance to your other plan on your own.
If your treatments are being covered through an MPI or WCB claim, we will need your claim number, date of accident/injury, and adjuster's/case manager's name if applicable.
Due to privacy policies of some insurance companies, we may not always be able to call on your behalf to get the information necessary to direct bill. If that is the case, you will need to call your insurance company, refer to your coverage handbook, or ask your plan administrator for more information on the following:

Prairie Fitness & Rehab offers direct billing to some private insurers, the insurance companies that we currently bill are as follows:
Manitoba Blue Cross
National Blue Cross
Canada Life (and branches of eg. Merit)
If your insurance company is not listed please feel free to ask us to look into it to see if its something we can try!
Prairie Fitness & Rehab accepts and bills claims through MPI accident claims. To do this, we require your date of accident, and your bodily injury claim number.
If you are not seeing any other healthcare providers for treatment, at any other clinic, you may be entitled to 23 athletic therapy visits, which MPI covers in full.
If you are seeing another health care professional for treatment, we will do an initial report and send it to MPI for review, and MPI will provide us with a decision within 10 business days. If there has not been a decision made within 10 business days we can begin treatment until your number of visits have been satisfied, or MPI comes to the decision that treatment is no longer supported.
Prairie Fitness & Rehab direct bills WCB for injury claim treatments. To do this you must have your claim number from WCB. The process consists of an initial appointment to assess the injury and fill out forms for a report that we will complete and send to WCB to request treatment approval for your claim,this process normally take 1-2 weeks. Once a decision is made regarding your claim WCB will contact us so that we can contact you and move forward accordingly.
